Common codes

HTTP	Code	Meaning
401	`missing_credentials`	No `Authorization: Bearer` header
401	`invalid_credentials`	Key invalid or revoked
403	`forbidden_scope`	Key lacks required scope
403	`forbidden_entitlement`	Your organization needs Inform + Operate (or a higher tier) for this endpoint
404	`not_found`	Resource not found (no cross-tenant leakage)
429	`rate_limited` / `quota_exceeded`	Throttled
503	`service_unavailable`	Identity validation unavailable (fail closed)
